Shipment Preparation

Supervisors must ensure that all healthcare workers in the lab who perform shipments must understand and follow these safety precautions when handling shipments of biological materials.

1. The primary container (Test Tubes) must be placed inside watertight secondary packaging.
  • Wrap frozen primary containers (Test Tubes) within cushioning material before placing primary container into the wide mouthed container (Plastic jar with red/orange lid). DO NOT add Dry Ice in the secondary container.

  • There must be enough absorbent material inside the secondary container to absorb and contain the entire contents of all primary receptacles. The secondary container should come equipped with an absorbent disk inside that will comply with this requirement.

  • When placing several primary receptacles in a single secondary package, customers must wrap the primary containers individually with cushioning material to prevent contact and possible breakage.

2. After you have packed Primary Container (Test Tube) in bubble wrap with absorbent disk in Secondary Container.
  • WARNING: Do Not slip a secondary container if Dry Ice is present. Dry Ice must be removed from secondary container (wide mouthed plastic container with red or orange top) before package can be shipped.

  • The container should be intact — no leaks or damage of any kind.

  • Close Secondary container tightly. Place complete unit into UN Approved fiberboard outer-wrap.

3. Place Dry Ice outside the secondary container.
  • Place the entire secondary packaging unit with the UN Approved fiberboard over wrap into the cooler box in the nested space provided. Substance must be able to securely hold the secondary packaging in the original position even if the refrigerant is lost.

  • Place 5 Kg (10 lbs.) of Dry Ice around the secondary packaging unit. Block or pellet form of Dry Ice is acceptable.

  • Place Styrofoam lid securely in place before closing and taping the outer fiberboard box shut.

  • All packaging must maintain its integrity at the temperature of the refrigerant, and the temperature and pressure(s) of air transport to which the receptacle could be subjected if refrigeration were lost.

4. Outer Packaging
  • The outer package must have adequate strength for its capacity, mass, and intended use.

  • Outer packages for infectious substances must be capable of passing the test requirements specified in 49 CFR, 178.609.

  • An itemized list of contents must be enclosed between the secondary container and the outer package. A properly filled out Test Request Form will fulfill these requirements.

ALL shipping paperwork must be completed and attached to the shipping carton(s) prior to departure from the pickup location. This includes the following:
  • FedEx Air Bill
  • Declaration of Dangerous Goods
  • Infectious Substance placard labels
  • “Danger — Do Not Load in Passenger Aircraft” label
NOTE: It is a violation to pre-label shipping cartons or packages (or placard vehicles) with a hazard class where the package does not actually contain material identified by the label. Imagine a first responder to a collision — seeing the hazard class labels — and the potential safety precautions that would be needlessly implemented.

ALL shipping carton(s) must be completely sealed and prepared for FedEx shipment (See Example “1”) prior to departure from location. Please consider the consequences if the driver were to be involved in a vehicle accident and the public were to be exposed to infectious materials.

The Federal Express Air Bill (Example #1)
Fill out the Federal Express Air Bill as you would any other air bill.

Complete Section #1 (From) with the Current Date, your company and address.

Section #3 Fill out the To: Section as:

  • In Section #4A (Express Package Service), check the FedEx Priority Overnight box.
  • In Section #5 (Packaging), check Other Pkg.
  • In Section #6 (Special Handling) check the “Saturday Delivery” if you receive prior approval from Monogram to ship on a Friday, ensure you check Saturday Delivery.
  • In Section #6 (Special Handling), under the section labeled “Does this shipment contain dangerous goods?” check the box that states “Yes as per attached Shippers Declaration.” Check the “Dry Ice” and record the quantity of dry ice included in the shipment. Check the “Cargo Aircraft Only” box.
  • Enter Total Declared Value = “$3,000”
  • Please remove the top “Sender’s Copy” and attach to a copy of the Shipper’s Declaration of Dangerous Goods form and retain for internal POD records.
